Full Medical Examination • Physical examination by Dr. Leem, a USCIS-authorized Civil Surgeon • Complete I-693 form preparation and certification • Review of medical history and conditions
Required Laboratory Tests • Blood test for Syphilis (RPR) • Gonorrhea screening • QuantiFERON-TB Gold test for Tuberculosis
Immunization Services • Complete vaccination record review • CDC-recommended vaccine requirements verification • Required vaccines available on-site (additional fees apply)

What to bring to your first Visit
Identification Card: Please bring a valid identification card such as a passport, driver's license, or any other government-issued photo identification.
Vaccine/Immunization Records: Please try to bring any vaccination records you have, including COVID-19 vaccines, TD, DTP, DTaP, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal, influenza, rotavirus, and meningococcal disease. If you don't have records of previous vaccinations, we can perform titer blood tests to determine your immunity status (additional fees apply). Most required vaccines are available at our office (additional fees apply).
Payment: We accept payment in the form of Cash, Zelle, Venmo, Credit or Debit card. Payment is required at the time of service.
